I, understand, agree my son/daughter is physcially able to participate in strenous athletic activity, and waive The Hoop Hall Dreams Camp, The Basketball Hall of Fame, Basketbull LLC and their officers and employees of any and all responsibility for injury or illness. I hereby authorize the directors of The Hoop Hall Dreams Camp to act for me according to their best judgement in any emergency requiring medical attention. I also understand that I am solely responsible for the payment of any such medical expenses and must provide The Hoop Hall Dreams Camp and Basketbull LLC with proof of medical and accident insurance.
I hereby grant The Hoop Hall Dreams Camp, Basketbull LLC and The Basketball Hall of Fame to any photographs, film, video, or other images taken of my child. I hereby waive and release The Hoop Hall Dreams Camp, Basketbull LLC and The Basketball Hall of Fame, and shall neither sure nor bring any proceeding against The Hoop Hall Dreams Camp, Baskebull LLC or The Basketball Hall of Fame for, any claim or cause of action, relating to the use and explotation of the pictures in The Hoop Hall Dreams Camp, Basketbull LLC or The Basketball Hall of Fame’s publications. The Hoops Hall Dreams Camp, Basketbull LLC nor the Basketball Hall of Fame have no obligation to utilize pictures of my child for any purpose.
I, understand and agree that I must provide the appropriate and required medical and liability waivers for my child so he/she may participate in the Hoop Hall Dreams Camp. I, understand the Refund & Cancellation Policy andthe Canteen Policy.
Below are our Medical Form and our Injury Waiver and Liability Form. Players must submit both forms by Check-In at Camp to participate. If these forms are not received that player WILL NOT be allowed to participate in camp. No exceptions will be made nor will any refunds be granted if these forms are not present.
Please download these forms and return to our Office as soon as possible. The last day we will be accepting these forms at our Office is June 15. Forms filled out after this date must be either scanned and emailed to us or brought to us in person at Check-In.
To reserve a spot at the Hoop Hall Dreams Camp your registration must include a NON-REFUNDABLE $250 deposit per week (Example: If you sign up for the Super Week it would be a $500 deposit). These deposits with or without Cancellation Insurance are NON-REFUNDABLE. Cancellation Insurance can be purchased at the time of registration for $75 per week. Cancellation Insurance is good up until 14 (fourteen) Days Before Camp. If you have purchased Cancellation Insurance and cancel outside of 14 (fourteen days) prior to camp you will be refunded the amount paid above the Non-Refundable Deposit and the Cancellation Insurance Fee.

At the start of each camp, parents/players will be allowed to drop off money in the Camp Canteen. The Camp Canteen money can be used throughout the week at Camp for food, snacks, beverages, camp t-shirts and other gear. Our staff will keep track of the money spent and at the end of the week at check out players will get the unused canteen money back in an envelope. If you do not collect your unused canteen money at check out that money will be forfeited.
